Projects may include partnerships with parents, students, community organizations, higher learning, etc., but only teachers and professional staff of the South Fayette School District may apply for a Success for the Classroom Grant. This cover page will be separated from the rest of the application as all grants are blinded for review.

Please provide the following information for every item you intend to purchase for this project. If some of the items you will use to carry out this project are being paid by using funds from sources other than the South Fayette Foundation (i.e., the school district, other foundations’ grants, etc.) please include these items but enter zero (0) for the amount. This will let the reviewers know that the project is completely sustainable even if the South Fayette Foundation is not financing the entire budget. Attach additional pages if necessary. Briefly describe any items that may be unfamiliar to the application reviewers.

Grant Narrative

Please answer each of the following questions. If more space is needed, please attach extra pages. Please avoid using names of specific teachers and/or schools in your responses as all grants are blinded for review.

1. Briefly describe the project/s, uses of equipment and or other information pertinent to this funding.



2. What makes this proposal request innovative? How will it enhance the curriculum?


 
3. Some of the money raised by the Foundation for grants is funded through tax credit donations. Does your programs meet the criteria of STEM/STEAM focused and if not, please indicate why this is not applicable.



4. Describe the student population targeted by this request. How many students will be served? Might the use of this equipment, program and or project be repeated with new students in future years? Explain.




5. What objectives or learning outcomes do you hope to achieve through this project?




6. How will you evaluate the success of this request? What data, criteria, assessments, surveys, statistics or other measures will you use to assess its impact?



7.   How will the Foundation be recognized in the implementation of your project?



8. Anyone rewarded a Success for the Classroom Grant must complete an evaluation report by the end of the school year in which the project occurs. This report does not need to be lengthy but it does need to address the following questions:

       * How did you evaluate the success of this request? What data, criteria, assessments, surveys or other measures did you use to assess the impact of this project?

       * How many students were served by this request year and how many do you expect will be served in the future?

       * If you plan to continue the use of this request with future students what, if any, changes will you make in the project?

       * Please complete a simple budget for your project. Was the entire grant award spent? Did your project require additional outside funding?

       * How was the South Fayette Foundation for Excellence recognized in the implementation of your project?

       * Please attach photos and any written documentation (i.e., journals, surveys, etc.) from your project. Photocopies are acceptable.

You do not need to answer these questions now. This is to advise you that if your grant request is awarded you will be required to submit a report at the end of your project that answers these questions.

These funds will cover projects to be completed through June,  2018
ANY AND ALL ITEMS FUNDED THROUGH THIS GRANT BECOME THE PROPERTY OF THE SOUTH FAYETTE SCHOOL DISTRICT.

YOU MUST HAVE YOUR PRINCIPAL AND MR. DERAMO SIGN OFF ON THIS APPLICATION BEFORE SUBMISSION OR IT WILL NOT BE CONSIDERED FOR REVIEW.
