Grade 1

1. Yearly Emergency Form

2. Physical Exam Form  by private physician if not submitted during Kindergarten
3. Dental exam by private dentist if not submitted during Kindergarten

Grade 2 

1. Yearly Emergency Form
2. Dental exam by private dentist during grade 2 or 3 

Grade 3

3. Yearly Emergency Form
4. Dental exam by private dentist if not submitted during grade 2

Grade 4

     1. Yearly Emergency Form

Grade 5

1. Yearly Emergency Form
2. Physical exam by private physician during grade 5 or 6.

Grade 6

1. Yearly Emergency Form
2. Physical Exam Form by private physician if not submitted during grade 5
3. Dental exam by private dentist during grade 6 or 7
4. Scoliosis screening conducted by nurse 

Grade 7

1. Yearly Emergency Form
2. Updated copy of immunization records for Tdap/MCV
3. Dental exam by private dentist if not submitted during grade 6
4. Scoliosis screening conducted by nurse

Grade 8 & 9

Grade 10 & 11

2. Physical Exam Form  by private physician during grade 10 or 11.

Grade 12

   3.  Updated copy of immunizations for MCV

New Students

1. Yearly Emergency Form
2. Copy of official immunization record (see immunizations)
3. Must provide a record of  previous dental an physical exams on entry and/or required at appropriate grade levels
